Provides information about and access to the following services and resources:
-Individual counseling and support groups
-Respite grants to unpaid family caregivers needing respite care (grants can be used to hire someone of your choosing to take care of your loved one while you take a break)

FAMILY CAREGIVER SUPPORT PROGRAM
Assists unpaid family caregivers who are caring for a person age 60 or older by linking them to available resources to make care-giving duties easier. Provides limited funds for eligible caregivers to seek respite care, adult day care, or items that make care-giving easier (grab bars, etc.).

Also provides respite care subsidies to grandparents age 55 and older who are raising grandchildren. Children must reside with the grandparent.

OREGON FAMILY TO FAMILY HEALTH INFORMATION CENTER
Oregon Family to Family Health Information Center (OR F2F HIC) operates a call-in center for family members of children and youth with special needs and chronic health conditions (developmental and emotional disabilities, behavioral problems, and chronic physical illnesses) and professionals who serve them. The center specializes in providing information and resources related to health information, health care access, and special needs systems and supports. All calls are confidential.

Families may also request to be placed on an email list to receive monthly correspondence about issues of importance to children and youth with special needs in Oregon.

Eligibility: Family members of children and/or youth (ages 26 and younger) with special physical, emotional, behavioral, or mental health needs, including those who are at risk or suspected of having a special need. The child/youth does not have to have a diagnosed condition.

Provides resources for family caregivers including paid respite services, medical equipment to make care giving comfortable and safer, one-on-one counseling, support groups, basic caregiver training, and Powerful Tools for Caregivers classes, which teach self-care.

- Adult family home
Community First Choice
-Provides attendant care and non-care related assistance services to older adults who would otherwise require nursing home placement
-Program participants can choose from whom they receive care services, including hiring family members as care providers
Community Options Program Entry System (COPES)
-Generally provided to clients who are also enrolled in Community First Choice
Medicaid Personal Care
-Provides personal care services for clients who do not require nursing-home-level care
Program of All-Inclusive Care for the Elderly (PACE)
-Offers all services through one provider.
-PACE can be funded either through Medicaid, Medicare or private-pay.
New Freedom
-Participants manage their own service plan and budget and take primary responsibility for obtaining services.
Medicaid Transformation Demonstration Medicaid Alternative Care (MAC)
-Supports unpaid caregivers of Medicaid recipients over the age of 55 Targeted Supports for Older Adults (TSOA)
-Supports older adults over the age of 55 who do not receive Medicaid Participants enrolled in one of the in-home care programs may also qualify for other forms of support including assistive technology and personal emergency response systems.

- $57,547 for a couple
- $123,600 for a couple when one is in a hospital or skilled nursing facility.
Clients whose resources exceed the resource limit may still be eligible if they expect to meet the limit within one month of applying for the subsidy.
Client must have an unmet need for assistance with two or more activities of daily living, or have a cognitive impairment and need assistance with one or more activity of daily living.
