Provides case management and facilitates access to available services and resources to eligible individuals who experience Intellectual and/or Developmental Disabilities. Service coordinators connect individuals to services which may include in-home support, residential services, supported living, employment and alternatives to employment, transportation, protective services, and other community resources and benefits.
