One-on-one case management support focused on eliminating students' barriers to attendance and academic success. This is an early intervention service for students who are starting to experience attendance, academic, or behavioral issues at school. Students work on an education and social support plan with their case manager and gain skills.
Education case management support for high school students who are academically behind and not on track to graduate. Case manager provides one-on-one support to develop an educational plan to graduate from high school or consider alternative options, such as GED. Case manager provides support and navigation on transition from high school to GED or alternative education programs. Case manager can also provide 3 employment readiness sessions.
