INTAKE PROCEDURE: Applicants can contact the caseworker they receive services from to request an application. Applicants will receive a PIN number.

DOCUMENTS: Application requires the last 4 digits of each household member's Social Security number or Tax Identification Number (issued by the IRS) and date of birth
