NEW EYES EYEGLASSES E-VOUCHER PROGRAM
Provides people in need with basic prescription eyeglasses through a nationwide e-voucher program. Please note that New Eyes does not provide or pay for eye exams.
Eligibility: This application is only for individuals who do not have a social worker, case manager or other health advocate. Serves all ages. Must have net monthly income at or below 250% of Federal Poverty Guideline, have an eyeglass prescription dated within the last 24 months, have no other resources available to pay for glasses, including federal/state programs or assistance from local charitable organizations, and have not received a New Eyes voucher in the past 12 months

    INTAKE PROCEDURE: Application should be submitted online by a social worker, case manager, or an agency on behalf of their client. New Eyes is also able to accept applications from individuals in financial need, who do not have access to a social service agency. Visit website for further information and for applications.
    FEES: As of April 1, 2024, there is a $10 Application Fee for all applicants.
