HIV+ CARE COORDINATION
Case managers work with HIV-positive clients to develop care plans that support the client's ability to sustain self-care. Care coordination services may include medical and pharmacy case management, respite care, in-home support services, benefits and health insurance advocacy, meals, and emergency financial assistance. Referrals may also be provided for social services, housing, dental, mental health and addictions treatment, employment, and financial planning.
