FAMILY CAREGIVER SUPPORT PROGRAM CASE MANAGEMENT
Case management and support services to unpaid family and other unpaid caregivers who provide regular care for a functionally disabled adult. These services allows for at-home care to continue, while family caregivers are provided with respite opportunities.

Eligibility: Unpaid caregivers who receive no form of compensation for care; caregiver and client must be age 18 and older. Caregiver must either live with care receiver or provide 40 hours or more of care per week.
