INTAKE PROCEDURE: Call for appointment. Due to a high volume of requests, new patient appoitments may take several weeks.
FEES: Accepts Oregon Health Plan (Medicaid), private dental insurance, and offers a sliding fee scale based on income and family size for those without insurance, and those with minimal insurance coverage. No one is turned away for an inability to pay.
