The Connection program provides these same services to deaf or hard of hearing individuals and their family members. Remote video conferencing, email, instant messaging, and information and referral to housing assistance, emergency food, and employment are also available.

INTAKE PROCEDURE:
Must be referred by primary care provider at West Salem Clinic or
Total Health Community Clinic
FEES:
Accepts Oregon Health Plan (Medicaid), Medicare,Tricare, accepts
most private insurance, and offers a sliding fee scale based on
income and family size for those without insurance, and those with
minimal insurance coverage. No one is turned away for an inability
to pay.
