INTAKE PROCEDURE: Call for information or appointment; walk-ins accepted during certain hours
FEES: Accepts the following OHP plans: Health Share/Care Oregon, as well as some private insurance. Uninsured patients paying on the day of service are charged $200 for general services. Labs and other tests may cost more.
