PATIENT AND FAMILY SERVICES
Provides financial assistance to children with cancer and their families through a Transportation Assistance Fund and an Emergency Assistance Fund. The amount of assistance is subject to change and dependent on available funding.

-Transportation Assistance Fund: Alleviates the financial burden of travel, meals, and lodging for families who have a child with cancer.

-Emergency Assistance Fund: Provides $300 per year in emergency assistance to families who have a child that has been inpatient or away from home for 15 consecutive days within the past three months. Assistance may be used for mortgage, rent, utility payments, child care, health insurance premiums, car expenses, or treatment-related expenses (e.g., meals away from home, prescriptions, and parking).

Eligibility: Transportation Assistance Fund:
-Ages 0-18 that have been diagnosed with cancer and their families and treated before 21st birthday. If child has a brain tumor, the tumor must be high grade (III or IV) or anaplastic to be eligible
-Child must be a US citizen or reside in the US with an I-551 card (green card) for 12 months without prior history of the current illness
-Guardian applying for assistance must have less than $5,000 in easily accessible bank accounts (such as checking and savings)

Emergency Assistance Fund:
-Same as for Transportation Assistance Fund and child must have been inpatient for 15 consecutive days during the past 90 days, or child must have been away from home/relocated for treatment for 15 consecutive days during the past 90 days
