CASE MANAGEMENT
Provides case management and wrap-around services to individuals. Support includes enrolling people in InterCommunity Health Network IHN-CCO, and obtaining identification, birth certificates, social security cards, and DD214 Forms. Can assist with employment related needs and supportive housing.
