CLACKAMAS COUNTY DISABILITIES PROGRAM
Provides case management and facilitates access to available services and resources to eligible individuals who experience Intellectual and/or Developmental Disabilities. Service coordinators connect individuals to services which may include in-home support, residential services, supported living, employment and alternatives to employment, transportation, protective services, and other community resources and benefits.

INTAKE PROCEDURE: Call or email to request information or assistance with applying for Developmental Disabilities (DD) services. An Eligibility Specialist will complete an intake interview and assist applicants through the eligibility review process.
