Disabilities who meet specific eligibility criteria. Applicants
must be an Oregon resident.

INTAKE PROCEDURE:
Call or email to request information or assistance with applying
for Developmental Disabilities (DD) services. An Eligibility
Specialist will complete an intake interview and assist applicants
through the eligibility review process.
