INTAKE PROCEDURE: Call for appointment
FEES: Varies by service. Accepts OHP, Medicare, and provide insurance. Also offers sliding scale fees.
DOCUMENTS: Tribal card or certification from a federally recognized tribe, birth certificate/photo identification such as state issued ID or a driver's license, social security card, insurance cards. Proof of income will be required for those without insurance. Klamath Tribal members or children of Klamath Tribal members residing in Kalmath County must provide a current utility bill or rental receipt with rental contract in order to show residency.
