Provides personal care to individuals so they may remain independent in their homes. Offers contracted chore services, personal care (hygiene), respite care, home health care, friendly visiting and can provide Lifeline Personal Emergency Response units (PERS).

Eligibility: Older adults and adults with disabilities in need of in-home services. Assistance for children with disabilities is available on a case-by-case basis.
Hours: Office: Monday-Friday 8am-5pm
