Chronic Disease and Diabetes Self-Management programs are evidence based 6 week programs, that meet 1 time a week for 2.5hrs via zoom and in person at select YVFWC clinic locations. The programs provide education and tools to empower participants to better manage their condition and lead healthier lives. Topics include nutrition, exercise, and communication.
