-Oregon photo ID or driver's license for all adults in the household
-Proof of each dependent child (2023 Form 1040 income tax return or 2023 Form OR-40 listing dependents) OR one of the following for each child in the home: social security card, medical card, or birth certificate
-Current utility bill for proof of address
-Proof of income for the last 30 days or proof of no income for everyone in the household
-Completed application
