Eligibility:
-Must be a United States citizen
-Must be an Oregon resident or intend to live in Oregon
-Have a household income at or below 250% Federal Poverty Level (FPL)
-Be less than 65 years old
-Must be uninsured or be underinsured
-Have been diagnosed as needing treatment for breast or cervical cancer, or specific precancerous conditions (listed in application guide)
Women who qualify do not need to be enrolled in ScreenWise in order to access BCCTP.

INTAKE PROCEDURE:
Prior to beginning the application process, the provider must
ensure that the patient meets the presumptive eligibility
criteria.
-Visit website and download the BCCTP Enrollment form
-Complete the form in its entirety with the patient
-Fax the completed form to OHP
