INTAKE PROCEDURE: Call for information and to schedule an appointment. Same-day appointments are available for established patients only.
FEES: Accepts OHP, Medicare, and most private insurances. Sliding scale fees available. No one turned away because of inability to pay for services.
DOCUMENTS: Insurance information, if applicable. Income information if applying for sliding scale fees.
LANGUAGES: English and Spanish; language line access is available for many other languages
