Eligibility:
Ages 16-25 with a mental health diagnosis, eligible for Oregon
Health Plan (OHP), and who have a recent mental health assessment
(within the past year) and an Individual Service and Support Plan
(ISSP) that identifies transition-related needs and goals,

INTAKE PROCEDURE:
Call for information and screening. Referrals can be made through
DHS, juvenile or OYA worker, therapist, school, community partners,
family member, or self-referral.
FEES:
Varies by service, accepts OHP
DOCUMENTS:
Insurance information, if applicable, current mental health
assessment, and Individualized Service and Support Plan (ISSP).
LANGUAGES:
English, Spanish, interpretation by arrangement
