MISSION: TRANSITION
Outpatient program for youth that focuses on developing skills to assist with the transition into adulthood. Services include case management, skills training, group skills training, and pro-social activities. Areas of focus include independent living skills, mental health symptom management, social skills training, academic improvement, job readiness, and health and safety.

Eligibility: Ages 16-25 with a mental health diagnosis, eligible for Oregon Health Plan (OHP), and who have a recent mental health assessment (within the past year) and an Individual Service and Support Plan (ISSP) that identifies transition-related needs and goals,

INTAKE PROCEDURE: Call for information and screening. Referrals can be made through DHS, juvenile or OYA worker, therapist, school, community partners, family member, or self-referral.
FEES: Varies by service, accepts OHP
DOCUMENTS: Insurance information, if applicable, current mental health assessment, and Individualized Service and Support Plan (ISSP).
