Care Coordinators send real-time requests for resources to community-based agencies on behalf of clients; monitor and track the outcomes of requests to services; and remain in communication with clients and agencies involved.
Eligibility:
Unrestricted; open to anyone seeking advocacy and support while
exploring information and referral about social services. Clients
do not have to be participating members of any medical insurance
programs.

Monday-Friday 8am-6pm
