INTAKE PROCEDURE: Call to schedule an appointment or walk in for urgent care
FEES: Accepts OHP (InterCommunity Health Network CCO), Medicare, and private insurances. All self-pay visits require an up-front $50 payment. Financial assistance and payment plans may be available.
