INTAKE PROCEDURE: OHP members must be referred by their Family Care coordinator, wraparound coordinator, or other OHP representative. Call the Access Department for more information or to make a referral.
FEES: Accepts OHP and most private insurances
DOCUMENTS: None required at intake
LANGUAGES: English, interpretation by arrangement
