INTAKE PROCEDURE: Call for information and to schedule an appointment
FEES: Accepts OHP and some private insurance (Moda, PacificSource). Sliding scale fees available. No child will be turned away from receiving vaccinations because of inability to pay.
DOCUMENTS: Immunization record and insurance information, if available.
