Eligibility:
-Age 65 or older, children age 0-5, pregnant or gave birth in the past 12 months, OR anyone with a qualifying health condition
-Have a qualifying living condition: leaving incarceration, or leaving a mental health or substance use recovery facility, or involvement in the Oregon child welfare system (now or in the past), going from Medicaid-only benefits to Medicaid plus Medicare, or young adults with special health care needs, or household income 30% or less of the average yearly income where you live and lack resources to prevent homelessness
-Have a lease or written rental agreement
-For hotel stays during home changes: Household income is 30% or less of the average yearly income where you live AND lack resources or support to prevent homelessness
