HEALTH RELATED SOCIAL NEEDS HOUSING SUPPORTS HOME MODIFICATION
Health-related social needs (HRSN) are basic needs that affect a person's health. These benefits help members who qualify with basic needs including rent payment assistance and utility payment assistance; tenant support; storage fees; and home changes for health and safety including cleanup and modifications.

Eligibility: OHP members who meet all of the following:
-Age 65 or older, children age 0-5, pregnant or gave birth in the past 12 months, OR anyone with a qualifying health condition
-Have a qualifying living condition: leaving incarceration, or leaving a mental health or substance use recovery facility, or involvement in the Oregon child welfare system (now or in the past), going from Medicaid only benefits to Medicaid plus Medicare, or young adults with special health care needs, or household income 30% or less of the average yearly income where you live and lack resources to prevent homelessness
-Have a lease or written rental agreement
-For hotel stays during home changes: Household income is 30% or less of the average yearly income where you live AND lack resources or support to prevent homelessness
