CACOON
CaCoon is a statewide public health nurse home visiting program that provides community-based CAre COordinatiON for children with special health needs. Children served in the CaCoon program have higher rates of immunization, annual well-child visits, and dental care visits; and make fewer visits to the emergency department. Since family members have a central role in the care of their child, all services are planned around the desires and concerns of the family.

Cacoon is part of Clackamas County Access to Preventive Health (APH). APH centers access and reduces barriers to care across services, to improve immunization rates, reproductive health, and maternal and childhood health and nutrition.
