A must read update for all Lane County Oregon Health Plan members

Here's a critical update from Trillium Community Health Plan:

Beginning August 1, 2012, Oregon Health Plan (OHP) members who formerly received benefits from Lipa and LaneCare, will receive their healthcare from Trillium Community Health Plan, the Coordinated Care Organization in Lane County.

Healthcare benefits for these OHP members will not change and there will be no interruption in service. The only change will be the name of the health plan that manages their care.

Prior to August 1, Lipa provided access to physical healthcare to OHP members and LaneCare provided access to behavioral health and chemical dependency services. As a Coordinated Care Organization (CCO), Lipa and LaneCare will join together to offer coordinated services through one health plan: Trillium.

Coordinating physical and behavioral healthcare will allow critical health information to be shared by all of the providers involved in a patient’s care. This information sharing, while kept strictly confidential as regulated by HIPAA, will improve communication, enhancing the overall quality and efficiency of care delivery.

The best news for OHP members is that as a CCO, Trillium will work to help them stay healthy and to better manage existing health conditions, including preventative care and chronic disease management.
